If you are planning to apply for FMLA/Disability, please contact your employer's Human Resources (HR), department for assistance. FMLA/Disability forms are unique to each employer. and your company will provide you with the form(s) they require AOC to complete.
To submit your form(s) for processing, please fax, email or drop off in person to our office.
- Please email our department with any questions, status requests, or new forms to be completed. The email address is: FMLADisability@athensorthopedicclinic.com. This is the preferred form of contact.

Once form(s) are completed, we will follow your instructions for returning completed forms. Note: There is a $20 fee for completion of FMLA/Disability forms which needs to be paid at the time of submission. You may pay this fee in person or call our office at 706-433-3128.
Please allow 7 business days to process your forms. We encourage you to get forms to us as quickly as possible, so we can return them to your employer before the specified deadline.
